FRED CARL DILGER III This is a new contract to provide expertise in overseeing ongoing and future radioactive waste and nuclear materials transportation planning, impact assessment, monitoring, and mitigation activities relative to the possible effects of a high-level nuclear waste repository or related facilities in southern Nevada and potential transportation implications of activities ongoing or planned at the Nevada National Security Site, formerly the Nevada Test Site. $200,000.00
3. LARKIN & ASSOCIATES PLLC This is a new contract to provide professional services by accountants as expert witnesses to the Bureau of Consumer Protection (BCP) in matters pertaining to the utilities, specifically natural gas, rate cases heard before the Public Utilities Commission of Nevada (PUCN). Services provided include: Analysis of the utilities companies' filings, preparation of interrogatories and participation in other discovery activities; assistance in preparation of responses to interrogatories propounded upon BCP by other parties; analysis of the company's direct and rebuttal testimony, assistance in preparing cross-examination and attendance during such cross-examination. $150,000.00
4. KEVIN P. BYERS, CPA This is the third amendment to the original contract, which provides forensic accounting services for cases pertaining to mortgage lending services. This amendment increases the maximum amount of the contract from $119,950 to $250,000 due to increased volume of caseloads. $130,050.00
5. GARY VILKE, MD This is the first amendment to the original contract for an expert witness to assist the Attorney General's Office in a current/potential lawsuit. This amendment increases the original amount from $25,000.00 to $50,000.00 due to continued case activity. $25,000.00
6. WASHOE COUNTY REGISTRAR OF VOTERS This is a new contract between the Nevada Secretary of State and the Washoe County Registrar of Voters, whereby the Nevada Secretary of State agrees to cover the costs for the services of Data Information Management Systems (DiMS), a division of Election Systems and Software, a provider of proprietary voter registration systems, to set up online voter registration for Washoe County Registrar of Voters, per agreement executed between Washoe County and DiMS. $86,005.00
7. ACS STATE & LOCAL SOLUTIONS This is the first amendment to the original contract, which provides for performance compliance reviews and collects unclaimed property from various holders and delivers the property to the Office of the State Treasurer. This amendment allows the state to utilize vendor services through its database of unclaimed property owner information ("MissingMoney.com" or "Services") for the purpose of assisting in owner reunification efforts and increases the maximum amount from $6,000,000 to $6,015,900 due to the service. $15,900.00

9. EIKELBERGER AWNING & DRAPERY, INC. This is a new contract to provide ongoing manufacturing, installing and or repairing window dressings in various State buildings in Northern Nevada on an as needed basis at the request and approval of a Buildings and Grounds designee. $25,000.00
10. FAAD JANITORIAL INC This is the fourth amendment to the original contract, which provides janitorial services for multiple State buildings in Reno and Carson City. There are two components to this amendment. First, due to contractor¿s service rate changes for the Carson City DMV Office, this amendment will provide for the early termination of the contracted services for that office (from July 31, 2014 to September 30, 2012). As a result of that action, the contracted services amount for the Carson City DMV Office is reduced by $134,662.12 from $352,409.52 to $217,747.34. Second, this amendment will increase the amount of contracted janitorial services for the Library & Archives building by $34,415.01 effective May 1, 2012, due to an increase of staff now located at this site. The total net change to the contract is a reduction of $100,247.17 from current amended contract amount of $767,064.33 to $666,817.16. ($100,247.17)

FIXEL, LONNIE DBA COIT SERVICES OF RENO LLC This is a new contract to provide ongoing carpet cleaning, green carpet cleaning, carpet repairs, carpet re-stretches, upholstery cleaning, tile and grout cleaning, commercial HVAC cleaning, area rug cleaning, drapery cleaning, and blind cleaning for various State buildings, as needed, upon the written request and approval of a Buildings and Grounds designee. $10,000.00
12. PENTAGON PLUMBING, INC. This is a new contract to provide ongoing plumbing services and repairs to various State buildings in the Las Vegas area on an as needed basis upon the request and approval of Buildings and Grounds designee. $30,000.00
13. XCEL MAINTENANCE SERVICES INC This is a new contract to provide ongoing janitorial services to the Purchasing Warehouse, Division of Welfare (Cambridge Community Center), and Department of Motor Vehicles (Donovan) located in Las Vegas, Nevada. This contract includes $7,500.00 for extra services, for use on an as needed basis and at the request and approval of a Buildings and Grounds designee. $243,240.00

GML ARCHITECTS LLC This is the fourth amendment to the original contract, which provides professional architectural/engineering services for the Civil Support Team/Weapons of Mass Destruction Readiness Center; SPWD Project No. 07-C27; Contract No. 4505. This contract decreases the maximum amount from $763,254.25 to $753,254.25 to provide credit toward construction administration services for the civil site improvements since a new contract for this service is currently being awarded on the Field Maintenance Shop project. 33. EMC CORPORATION This is a new contract to provide ongoing continuous coverage for routine, preventative maintenance, and timely repair services as needed by EMC Corporation for EMC DMX3 arrays located in Northern and Southern Nevada. $320,137.85
34. EUREKA COUNTY TREASURER This is a new revenue contract to provide SilverNet services for the County of Eureka. $11,720.88
35. OMNICARE PHARMACY OF LAS VEGAS This is a new contract to provide pharmacy services to the Nevada State Veterans Home, a 180-bed skilled nursing facility. $400,000.00
36. ELIZABETH R. KOSIER This is a new contract to provide content for a five year plan that provides statewide programs and services developed and delivered as part of the Nevada Library Services Technology Act (LSTA) plan within the state. The Nevada State Library & Archives is required to develop a statewide plan for the LSTA program every five years in order to be eligible to obtain federal Library Services and Technology Act (LSTA) funding. $17,494.50
37. ACCESS TO HEALTHCARE NETWORK, INC. This is an amendment to the original contract which provides compensation to Access to Healthcare Network, Inc. for share office space for one staff member in the Office of Consumer Health Assistance in Elko. The amendment extends the termination date from June 30, 2012 to June 30, 2013 and increases the maximum amount from $6,000 to $12,000 to allow for an additional year of rental space and shared services. $6,000.00

ADDUS HEALTHCARE INC DBA This is the third amendment to the original contract which provides in-home care services for persons with disabling conditions and frail elderly clients of the Aging and Disability Services Division (ADSD) allowing them to remain independent in a community setting in lieu of institutionalization. This amendment increases the maximum contract amount from $348,640.00 to $2,140,087.00. The increase is necessary due to additional services being provided to an existing client base of the Personal Assistance Services Program (PAS), added to the contract by amendment, in June 2011. $1,791,447.00
